I always use a tap wrench when using an easy out extractor, using an adjustable wrench makes it more likely for the extractor to wobble which is how they break. Easy outs are hard as hell but brittle so they can break pretty easily if you're applying uneven force. And breaking your extractor off inside the broken bolt is a nightmare scenario.

The smaller the hole, the smaller the diameter of the extractor, which means higher torsional stress. If you consider that the piece being extracted needs a certain amount of torque to move, the larger the extractor you can use the better your chance of success. However, the larger the hole, the more accurate you have to be to avoid drilling into the threads, which will make using the extractor impossible. At that point, your best bet is to drill the hole oversized and use a threaded insert.
