Thanks Mike, I hadn't played with this feature yet. So just distribute a copy of the PowerPoint with the embedded Form(s) through a Teams assignment! I'll try that today!

I do not have this "Forms" icon in ppt 2019, how i can add it?

Are forms visible when using PowerPoint Live? I am sure I saw one of your videos where you showed this off? So in Teams your participants could fill in the form embedded in your PPT.

Thank you! I successfully inserted a quiz into a slide, however, when I present and scroll - it advances to the next slide instead of scrolling through the form (it is multiple pages). I have Advance Slide On Mouse Click feature unchecked for all slides. In presenting mode how can I scroll through the form and not scroll to the next slide?
