The first step in the formation of a neutron star is the collapse of a massive star. When some of the most massive stars in the universe run out of fuel, their cores begin to collapse under the influence of their own gravity. As this happens, the core becomes increasingly dense, and the temperature and pressure inside it rise exponentially.

Eventually, the core will have collapsed so much that the electrons are forced to combine with protons, forming neutrons. This process is known as electron capture, and it releases a large amount of energy in the form of neutrinos. These neutrinos will provide the energy necessary to drive the explosion that will occur later in the process.

As the core continues to shrink, it becomes increasingly unstable, and the pressure and temperatures inside it continue to rise. For anyone that wants further explanation,

The neutron star showed in the shirt is a PULSAR, pulsars are basically the next evolution of a neutron star; all pulsars are neutron stars but not all neutron stars are pulsars. Pulsars can be identified from the beams of light that shoot from their poles; a regular neutron star will just be a ball.
