Build Canvas PowerApps Based on Dataverse Table

This video will demonstrate the process of creating PowerApps based on Dataverse table which contains lookup, choice and date columns.
It is very easy to create PowerApps using Dataverse table. One can select Dataverse as a source while creating Canvas PowerApps. This will auto-provision the PowerApps for you with three screens (Gallery view, Detail, new/edit).

Thanks it canvas app worked using Dataverse. up to the point of creating a new record. Ii can even edit an existing record but I can not create a new record.

When I save it I get a fault. Network error. Field forms_name is required. I've tried everything for hours. No forms_name column exist. Any ideas?
