The issue of heat shields falling off Starship rockets has been a painful problem for SpaceX for a long time.
Although the company has spent a lot of time, money, and effort to research and handle this problem, this situation continues to recur.
And it looks like it reached the tipping point in the event of November 18, given that many tiles were missing or damaged, raising massive concerns about the rocket's ability to withstand the extreme temperatures of atmospheric reentry.
Especially within the context, after IFT2, Starship is set to join a soft landing test in IFT3 which is related to the thermal protection system.

I think only long term answer is to find a different solution. Tiles have always been a dicey problem on every vehicle they have been used on. Shuttle Columbia proved that even after decades of work a tweaking and studying, they never really engineered out the risks. Then, you have Dream Chaser that is years behind because of how complicated the tile application is on that craft and we still do not know 100% how it will perform and if they will also lose tiles in real operations. I just wonder if the tiles are really the solution SpaceX wants, or if someone in a Government Agency is telling them “it’s the only ‘proven solution’ if they want to keep testing. I have seen some analysis that says because of the size and shape of Starship, the Stainless might not need that much protection to begin with. It wouldn’t surprise me if SpaceX is expecting the tiles to fail, but for Starship to survive without them just to make a point to everyone insisting they needed a tile shield.

Hey, NASA's Space Shuttle Columbia was lost due to a hole in the leading edge of the left wing. The carbon composite material in that leading edge was struck by a 1.5-pound piece of thermal insulation foam that came loose from the External Tank about 10 seconds after liftoff.

The rigid ceramic fiber tiles had nothing to do with that disaster. The Space Shuttle was launched 135 times, 133 times successfully. Those tiles performed exactly as designed during the 133 successful entry, descent and landings (EDLs) and prevented damage to the aluminum hull of the Orbiters.

It would be interesting if SpaceX could make the thermal protection in the form of a spray-on coating.

NASA should scrub the SLS and redirect that money in research for a solution to eliminate tiles.

Those are not stainless steel inserts for the new smaller tiles, but there are steel mock-up of the new tiles.

Good video, it was one of the more in-depth looks into the TPS that I've seen. Another matter however, may I humbly ask of you to say the Fahrenheit equivalent to the Celsius numbers you mention? Actually giving the Imperial numbers alongside the metric numbers across the board i.e. meters, kilometers, etc., would be greatly appreciated. I'm too old to relearn a whole new system and there's no way I'm alone. Thanks.

Can't they just make the reentry last longer, so that they would be no need for a strong heatshield ?

Can't SpaceX install the Tiles then Spray a High Temp Coating over the Tiles like they do other space ships? like the new Dream Chaser? This Idea might work?

On one space shuttle flight 700 tiles fell off so not anything new here. The shuttle didn't fail due to missing tiles it failed b/c a hole was punched into the leading edge of the wing by foam falling off the shuttles huge fuel tank

O tapete térmico com velcro é uma boa idéia e ajuda a absorver vibrações! Olha uma pata de lagartixa no microscópio também é interessante pois elas andam nas paredes! Usaria o Falcon9 com asas e escudo térmico para fazer testes! A NASA já tem um escudo térmico desenvolvido! Não sei como relação a direitos de projetos e si poderiam ser copiados!
