Painful Arc Sign – Special Test for Shoulder Rotator Cuff Impingement

The painful arc sign is a special test for shoulder impingement that is easily performed. When someone has rotator cuff pathology, often times they can actively elevate their arm overhead, however, they may experience discomfort in the mid range of motion between 60 and 120 degrees. This tends to get better as they elevate higher for those with rotator cuff inflammation.

This test has been researched several times in the literature with varying sensitivity and specificity. I have found it to be very sensitive to those with acute cuff inflammation. I don’t use this as much for testing a rotator cuff tear.
