I think someone made a GMod video based on that. The Sandwich had a little Russian hat and was the angel on Heavy's shoulder, who slowly became a demon (perhaps Lucifer?) and made him kill his pals, to the point where Spy had to meat-shield and Dead Ringer his way through it all. One of the comments said that this is why they make their Heavy eat chocolate bars.
