Cortana was metastable. She finally coped with her rampancy and was able to gain peace and true sentience. Her "body" in games Combat Evolved through Halo 3 was just a projected image. She even stated in 4 that she couldn't really see or feel the sun, she just knew. When she protected Chief she finally accepted what was happening. Also she actually went through the stages!(melancholia, anger, jealousy) She then could see him and for the first time she touched him. An image can not touch or see.

This break-in was heavily influenced by the fragments. Remember how Wash said that the A.I. are what convinced their agents to try to rescue Church? Now Tex is obviously acting on her own, but she counts for actually being an A.I., Theta is the one who pushed for North to help, and Delta probably played a part in convincing York. Sigma's controlling Maine, Omega is almost always controlling someone, and they and Gamma were the ones torturing Church as well. They are controlling the game.

I always figured that as more fragments were added, they'd eventually start merging together to become someone closer and closer to the Alpha's original personality. After all, Sigma's whole goal was to become metastable that way. Either way it'd be a real interesting process to watch, and I for one wouldn't mind a miniseries focused on the Meta just for that.

Epsilon is the fragment that carries the memories, but Tex is the personality subroutine based on them. For instance, the Delta fragment contains the emotional core of logic and a personality subroutine based on that core. Tex is just the subroutine. Church later split himself into two fragments, giving his memories to one, but another personality subroutine based on those memories did not come into being, so Epsilon and Alpha are running the same subroutine and are the same person.
