➡️ PROBLEM:
An intubated patient is receiving mechanical ventilation in the SIMV mode with the following settings:

• VT = 400 mL
• Rate = 12 breaths/min
• Peak inspiratory pressure (PIP) = 40 cm H2O
• Plateau pressure (Pplat) = 20 cm H2O
• Inspiratory flow (V ) = 60 L/min or (1 L/s)

The patient is breathing spontaneously with a spontaneous rate of 12 breaths/min and a spontaneous peak inspiratory flow of 30 L/min or (0.5 L/s). What is the level of PSV that is needed to overcome the imposed work of breathing?

➡️ SOLUTION:
PSV = [(PIP - Pplat)/ Set insp. flow] x peak insp. flow
PSV = [(40-20/1)] x 0.5
PSV = 10

🟩 The calculated PSV level to overcome the imposed WOB for this patient is 10 cm H2O.
