i think the word you're looking for is "shoulders"

The marks on his shoulders are always there, sorta part of the default character model, hell you could walk around with no cargo at all and you'll still have the marks on your shoulders

He actually has those even if you walk around with nothing, though someone was saying it's a gradient situation where it gets more pronounced if you wear heavy packs, just from my experience it doesn't seem to make any difference

Man, I fucking love Death Stranding. There really will never be a proper sequel to it or a followup though because actual artistic games are poison to the industry as is today.

Those are shoulders, not arms. They're the part that connect the arms to the torso.
