I used to work in a recycling plant and we had a similar system... when we were going to clean after shift the main switch was locked with everyones personal lock so it couldnt be switched back on without everyone definitely being away from under the machinery and unlocking their padlock from main switch.

Had a skilled trades go home without removing his lock.
Took us over a hour by the time we got all the responsible people involved.
Team lead, supervisor, module manager, safety and the head of security (they have a master key)
There was a search for the trades tech, everyone had to confirm he had left the premises. This was not a lockout box but a power switch for a robot.

I had my lock removed from a heavy machine by a supervisor. they refused to sack him. I quit, it wasn't worth dying for that job.

LOTO box. Lock out, tag out. Definitely a lifesaver, too many people have been killed while performing maintenance by a coworker innocently trying to get their job done.

In familiar with LOTO procedures and I can’t believe how many young people really don’t understand the importance of it and has resulted in some injuries in my previous workplace. I worked at a food related industry, and we used so many different machines: RISCOs, VEMAGS, different belt lines, huge ovens, even a very large mixing machine that’s the size of like 12 people. One time, a sanitation worker was cleaning out the mixing machine to prepare for another order that required the use of a different ingredients, transitioning from seafood to poultry. The sanitation worker unfortunately decided not to Lock-Out Tag-Out the energy source of the machine, as he found it easier to have it energized to rotate the blades and to clean them. Unfortunately that mindset cost him two fingers when the machine energized and cut his two fingers off. The whole area was closed while HR conducted their initial reports, as well as filing an incident report with OSHA, and the dude being transported to the hospital. He did ended up losing those two fingers.

I had this exact system at my last job and it was brilliant. Everyone's lock came off except one. It was a millwrite who wasn't completing a task and so it resulted in EVERYONES locks going back on the box and supported in completing the task. If it was any other system I'm convinced it would have lead to something catastrophic

This literally needs to be more of a standard than it already is like this in all job sites in general needs to be a requirement, especially if it includes someone cleaning or working with a machine that could be very deadly or infect injuries if turned on during the process of them working on it.

I used to work on a team that repaired and maintained factory assembly lines. We used these to make SURE everyone was done and away from any kind of machinery.
We need to KNOW everyone is done before we fire up the lines.... otherwise someone's going for a hell of a ride.

Lockout tagout is very important, and it has saved a lot of lives. I had the unfortunate chance of witnessing a failure of lockout tagout where someone lost his life due to electrocution.
