It's difficult when you have been numbed up and they ask you to bite down. Because you can't feel your teeth so don't know your biting down weirdly lol

It's because the context is someone is looking very very closely at your teeth and biting down the wrong way is intuitively what you would do to show off as much of your teeth as possible with your mouth closed

You should just tell patients to say cheese as if they were having their photo taken
