How Clara/miss Afton actually died:
So Clara or miss Afton died after a fight with William, it was after Elizabeth died, she got into a fight with William, they was both hurt so Clara packed her stuff put in in her car and drove away to move somewhere else, she forgot to divorce William and started thinking about what would happen, she tried turning around but then she accidentally drove off a cliff, but in some AU's she died by ballora the ballerina or William killed her.

Anyway, William didn't actually kill Miss Afton. As we know from Five Nights at Freddy’s: Security Breach, they got divorced and then won a custody case (of the children) which William won unfairly. After that, he either killed her or she committed suicide (which is more likely).
