I think Kylo Ren had it right. When he tried to imitate Vader, the less like a human being you act, and the more otherworldly, you appear, the greater your intimidation factor. Wearing a mask puts a barrier between you and the outside world, and that barrier, that unknown element, inspires fear.

someone who has starved for just shy of 3 Weeks, I can tell you, it's extremely painful, you get abdominal pain after about the 6th day, after 2 days you start getting cramps in your belly.

This explains much to me about Vader, not only was he in pain due to the suit and sideouses cruelty, but top that with eating liquid slop once a month, no wonder he seems to never have a second to think back on Anakin with the myriad forms of pain he's going through, plus desperate hunger .. I'm surprised he didn't jump out of the auots helmet and cannibalize rogue stormtroopers(Now that would make an interesting new comic !)

The suit might add to how terrifying he seemed, but he was also very powerful and not afraid to use that power to nefarious ends. They may not fear him at first, but they soon would. Not to mention his reputation.
