At UT Medical Center, our case management team is critically important to us as they support high quality, efficient care delivery for our patients. Case management is a complex and multifaceted process. Case managers are responsible for the assessment of a patient’s physical, mental and social needs and abilities in order to ensure a safe and effective discharge. Our team assess over 3,500 patients per month for discharge planning needs.
