A guy at a welding/machine shop I worked at (large industrial type welding and machining, like 10 foot swing lathes and mills with 40+ feet of travel) dropped an 18 ton casting (a Terex 450 rock crusher main body) while trying to flip it over with a gantry crane, we felt the impact in a seperate building over 300 feet away, it cracked the 2 foot thick concrete slab it fell on and left a 6 inch deep gouge in the concrete, the dipshit ran his nylon straps over a freshly machined edge that was sharp as a razor, instead of putting the cut protection pads over the sharp edge to protect the straps like he was supposed to. If it weren't for the fact all the machining was internal and the casting wasn't damaged or warped after the 4 foot fall (that's an 18 ton steel casting for you) the guy would have been fired, but our boss was livid regardless, he stopped work throughout the entire facility and made everyone come to a safety briefing on safe rigging practices, using the guy's F'up as a prime example of how dangerous rigging failure can be. That casting would have turned him into a human smoothie if he had been a few feet closer to it...

Looks to me like
A: wrong capacity or damaged slings
B: Nothing to buffer the sharp edges of the cylinder bolt holes.
C: Probably most importantly the operator started to trolly out too soon. You can see the the cables didn't maintain a verticle attitude. When that occured and the load started coming down the cables got slack in them. Then it's fate was sealed as the load was essentially falling and the slings couldn't take the shock loading. One could have saved it but you have to see the problem and react very quickly.
Playing around with the boards could have contributed to the accident, perhaps distracting the operator. Better way would have been to place sling/s on the floor and repositioning afterwards.
Where I worked if we dropped a load it was bad juju. There would be safety stand downs and meetings and the operator would be in trouble. Every department involved would have to go to meetings. I never dropped a load knock on wood.

Working in the high bay lifting about 5 tons everything was calibrated and certified and proof loaded and properly secured. One of the slings broke and we got five tons swinging through the are 8 ft off the ground. Fortunately it's swaying away from me. It gets your attention real quick. My point sometimes things just break. That was at Lockheed Martin.

There was slack in the line as it hit tipping point. Should have had a come along or something to trip it with mechanical means. That would’ve prevented shock loading the rigging.

Simple case of shock load to the rigging gear. The piece hit the tipping point at the same time the crane slacked, causing the sudden "jerk" onto the slings. The wood blocking to aid in tipping also failed, causing the workers to tip it by hand while the gear was slacked. Very dangerous and, unfortunately, very common rigging event

Straps are good for static loads. If it jerks it will probably break.

The operator was in the most danger, I'm glad he was paying attention to see that Gantry hook swing at him! I've been unlucky enough to almost lose a hand to a chain, thank God no major damage was done and any wounds were easily sewed back. I was only left with minor scars but that's the day I really learned to respect chains, straps and hooks. I moved 2000 lb palm trees all day so I'm using the same nylon straps. If you're not careful they can be slick. Any kind of break in the strings and they can snap without warning. Always keep your straps in good condition and pay attention to where the seam is. Something I've been telling my guys since I was 14 all the way until now (I turn 38): "if we're doing something dangerous stop what you're doing and picture yourself dying. You will always do the opposite of what you saw cause you're death."
The reason I say this is I have walked away from so many incidents that could have been fatal because I took 5 minutes and thought about what would happen if I stood in a certain spot or I moved anything. When there's a 2000 lb palm tree dangling above your head and your machine can barely hold the tree in the air you should always know where your bail spot is.

OSHA probably was mad because somebody almost got injured/killed, luckily no one was injured or killed, this is an example of why you have to be careful at the metal recycling center or what it's called.
