Yo, whoa! This was actually legit along the lines of how you're supposed to answer her if you want to live. The legend goes that a woman with a surgeon mask will walk up to you and ask if she's pretty: if you say no, she kills you, but if you say yes she'll remove the mask and show that her mouth is cut open like a smile. She'll ask again, to which if you say no she'll kill you and if you say yes she'll cut your mouth to look like her. If you just try to run away, she'll follow you home and kill you there.

The way to get around this is to basically answer ambiguously or distract her. One response I heard was: "Eh, so so, " which will make her consider the answer and force her to think about what she has to do to you, in turn giving the opportunity to escape without being followed. Another response is to basically go "I'm sorry I have a prior engagement I need to get to." This will, ironically, have her apologize to you sincerely and allow you to leave. Toji's answer, which is basically saying "You're not really my type, " might also be ambiguous enough to confuse and distract her since he isn't exactly saying yes or no to whether she is pretty. Another response similar to his, that I've heard, is saying that you have a girlfriend/boyfriend, which makes her also apologize and leave you alone.

And remember, Toji can't even see curses. This is his heighten senses that let him see a world where he can see more than anyone can (Gojo's Six Eyes being the sole exception)

Edit: honestly, some of y'all don't understand what 'heightened senses' mean. Heavenly Restriction like Toji and Maki takes away all of their CE and ability to see CS. But in exchange gives them heightened senses which inturn allows them to see a world different from others. It is due to this ability that they can 'see' the curses

For ppl who doesn’t know about this women named( kuchisake onna) here is a Japanese legend called slit-mouth from hein era. Her husband slotted her lips while on a April fools day she tried to joke a cheating scene with mannequin. She would roam around n ask everyone ‘if she’s pretty or not’ in the alley or dark places with a mask on. If they said yea she would remove the mask n ask if she was still pretty. Later she would kill the person. Whoever said yes or no before her removing the mask would be killed however there was a way to confuse her by telling that (maybe or u r okay or not my type) this is not a accurate answer to her n the person could escape the bad consequences. This is exactly what toji did. Confused her by not answering her question.
