Synchronous and Asynchronous Transmission

Looking at and evaluating two ways we can transmit data, which are based on how clocks can be used differently to regulate the transfers. There start and stop bits used in asynchronous communication are explained.

QQ - Is a cluster's heartbeat packet considered as an example of Synchronised transmission ? Cause every 2 seconds it is going to check if all the other nodes are alive or not.
Additionally, to synchronise both the sender and the receiver, what is the device used to spike or down that voltage at the same time ?

I Don't really understand what is that clock that you talk in synchronous transmission, is that another wire different than the wire where you transmit the data ? how does the clock work, I mean it sends a periodic signal and that way the receiver can synchronize ?
