For those unfamiliar, this is a clip from an episode of Girls. His character (also called Adam) is talking to a female character on the show Jessa who was freaking out all because the baby had (slightly) spit up down her back and she couldn't reach to clean her shirt off herself. Adam couldn't help her as he was literally holding the baby (his niece) which Jessa had essentially thrown at him after the spit up happened. Jessa was massively overreacting and shouting at Adam for his lack of help to which he calmly responded back to her with the dialogue you hear in this clip. The fact that she can't clean a little baby spit up off her shirt herself without completely losing the plot and not asking but rather demanding help is a metaphor for her entire character. Jessa (along with the other female characters on this show) is a whiny, manipulative basket case who encompasses all the worst characteristics of the (stereotypical) spoiled and entitled Millennial. Hope that helps.
