About the journal, it might be referring to Sam. I mean, look at his head. It is torn apart meaning there is something under that head costume. The Good Sam inside that suit.

I really like ur theory, and i would like to say 2 things.
1. Dr Devlichi was talking aboout thefirst skin, Rachna, or smth like htat, the one that opens up its head into 2 parts during the stun and jumpscare aniamtion. I thought that it was implied in the note that he was aying that it represents Anger and Happiness, and all the other oppsoites.
2. I think that sam might also feel the pain of the otrches, if his soul is stuck inside the game.

If you don't know, the note, as seen in the video IS in fact referencing duocara.
Not the one Eddy made, one he made.
The mayor one.
As far as I know, the season always has the skin that they are talking about, and duocara is the only thing that could be made.
But this could be talking about Rachna, as she has 2 sides of her face, and she is horrifying.

If you think about it, I think Mayor duocara is a whole different character because when u get an alt skin it normally appears with the original skin, but this time it doesn't. So maybe the notr is talking about Mayor Duocara
