I underwent FFS (Facial Feminization Surgery) with Dr. Cardenas three years ago. Given my pronounced masculine features, my expectations were quite modest. It took me about two years post-surgery to fully appreciate the results, considering both the lingering swelling and the effects of HRT. Now, comparing my before-and-after photos, the transformation is genuinely remarkable. I no longer have difficulties passing, and this has immensely elevated my confidence and self-acceptance. I have FFS at the end of August. I'm a little scared, because it's surgery, but I'm mostly excited. Thank you for this video. Now, I understand that I shouldn't get my hopes too high. Any feminization will be welcomed!

Thanks for that. I just had a consultation today about FFS and actually to my surprise the surgeon recommended only doing the forehead and nose and nothing else, despite me having various ideas about a bunch of other things that need to be done. The surgeon simply said nope, I don't agree, you don't need that, nor that, trust me. So I really appreciated how he actually *wasn't* being the sort of sales person you mention here in the video, and that makes me trust him.

That said I'm still probably only going to do the forehead first, and wait with the nose till a year or so later when I can already see the results. Partly because I'm afraid of changing too much at once, and part of it is I think it's a bit like you said, it kind of would be like throwing all the eggs in one basket and saying "ooh great after this surgery I'll be 100% the woman I always wanted to be." which is setting yourself up for disappointment. It feels much healthier to take it slowly, step by step, and appreciate every part of the journey rather than only hoping for some dream end result that might never come. It will also mean doubling the recovery time but at least it won't be as traumatic each time, a slower and more gentle transformation.

Thank you for saying this... it's on the mark. I was fortunate enough to have had FFS many years ago and it was a profound part of my being able to transition (even though I have highly mixed feelings about many aspects of my surgical results). The surgeons' skills vary widely and everyone has to be realistic about what can and cannot be accomplished through it. It's one step of many but not a magic pill which will answer all your dreams. I would suggest the same about transition in general.

I had my surgery in 2019 by (Dr. Bart, Antwerpen, Belgium ) it has changes a lot some people don’t recognized me lol 😮

Thanks for this video. Now I do think future technology will allow people to literally pick and choose your appearance, but for anytime in the foreseeable future, have realistic expectations. FFS will improve the situation by making someone more feminized, but it isn’t quite the Star Trek level tool we wish it were quite yet.

I really love this video. Having realistic expectations is a really good approach to facial feminization surgery. I am leaving on Saturday to Boston to have my facial feminization surgery with Dr. Spiegel on Wednesday. I have been working my tail off and waiting for the surgery for a while now. They are doing quite a bit of work and I am nervous about the surgery but I’m also ready. I have written down all of my questions and concerns for the surgeon. My biggest fear with the surgery is that I will look deformed. I know that sounds ridiculous but when you’re used to seeing one face when you look in the mirror and you’re going to have surgery to change that, it’s very scary actually, even though you may want it. I also have a fear for some reason that the surgery just won’t work for me. I know that is so silly but I don’t know. I’m sure the surgery will turn out just fine and I am very fortunate that I have my family going with me in support. I have my best friend waiting for me when I get back from surgery and I am in a really good place in my life. Anyway I loved this video and I am going to go into my facial feminization surgery having realistic expectations. I don’t expect to come out of surgery looking like a totally different person but at the same time I know that the surgery will soften and feminize what is already there. Thank you.

Hi Dr. Z! I am planning to get FFS in the future an would be ecstatic even if all the surgeon could do is soften my looks a little bit. While sky's the limit feminization would be wonderful, being able to just tolerate looking in the mirror would be fantastic. When I use a mirror, I only look at a small portion of my face at a time and, even at that, I tend to get really dysphoric. Putting on makeup takes a hell of alot more time than it should - Lol! I would really love your advice on vocal feminization. I have made an appointment with are vocal feminization coach she is not ready to work on it with me yet that is fine but my question to you is. I would love to get a picture in my mind as away to work on it but I am finding it hard to do this maybe it is because I want it so badly what do you think or do you have any advice

Excellent Video Dr Z. I have heard about transition that it is at best like immigrating to a new country. You can blend in pretty well but the locals will always know that you are not native and not 'one of them'. If you are polite and try to pick up the relationship and dialogue cues and blend in fairly smoothly you can be accepted and have a meaningful life in your new country. Some trans women may encounter signals of non-acceptance that can trigger fears, frustration, and intense dysphoria. Unfortunately, sometimes being militant and insisting on inclusion can seem like a colonization effort instead of an immigration. I really enjoyed your other video about what we can learn by being mis-gendered. I think it dovetails nicely into this video on facial feminization.

I’m a trans woman who began my transition at the age of 48. I’ve watched a great number of videos on FFS and have noticed that the majority of people who get dramatic results are in their very late teens or, most often, their early twenties. Testosterone has had a LONG time to affect my facial structure, so when I imagine what can be done, I look in the mirror and try to simply manipulate the soft tissue very gently and rely on that as an example of what is possible. I know that some bone structure Mac be altered, particularly brow shaving and rhinoplasty, but my goals are based more on the adjustment of easily manipulated soft tissue over the overlying structure. I feel like this is a more realistic way of setting expectations. Do you agree or recommend any other way of setting a realistic expectation?

I agree we all have hopes and sometimes false expectations, furthermore most doctors will show you photos of their work, but they only show what we want to see. I have talked about that even with my plastic surgeon, but my guess is that it doesn't matter what we hear, we will always pick our wishes rather than being realistic.

I think for me, being at the beginning of transitioning, I wouldn't want to even consider FFS until after 2 years of HRT. Not that HRT will change the bone structure, but it will change fat distribution and muscle tone (as well as a few other things), and that may be enough for me, but I won't know until after be on HRT for a while. Also, even though I'm not very practiced with it yet, makeup can do quite a bit when in public, and that too, in addition to HRT, may be enough for me. I think the most important concept to keep is: "If after ample research you find it to be right for you, then great! Go for it! But, Don't rush into anything."
