Earlier I too used to give only methylcobalamine and didn't used alpha lipoic acid .. but it really makes difference in these patients (patients on pregabalin and/or duloxetin) not a major but gives a better outcome when added to methylcobalamine. However it depends completely on manufacturer also. Alpha lipoic acid prep from Nurokind / meganeuron od always work better than only methylcobalamine. Brand too have different role, if you use generic of same companies will still doesn't work.

Thank you for the one more wonderful video sir . Sir what’s the exact mechanism of action of pregabalin in neuropathic pain.? Literature says it’s at multi levels from brain, spinal cord, periphery .Does it stops Demyelination of neurons sir? I am confused how to explain my patient it’s major action in neuropathy. Thank you very much sir .🤝🙏💐

Dear Sir, A nice video. However, the monofilament test is known for Loss of protective sensation ie touch sensation is noted when applying 10 gms pressure. There is evidence to say that good control of Diabetes with insulin reduces painful neuropathy. Correction of the internal milieu is important. The studies with Alpha lipoic acid in reducing autonomic dysfunction when given parenterally are noted. thank you for a nice recap.

Sir... One doubt is that...when should we add the 2nd class drug after the 1st class drug?
When we have given max possible dose of first class drug and there's no response... Then should we add 2nd class drug?
Pls guide sir... 🙏

Resner plus is one of the best drug combination to treat diabetic neuropathy. But remember this drug combination usually cause dizziness, hypotension.
