When Stormfront confesses to Homelander, she says Fredrick Vought gave her the " first successful V injection" It is likely that the V that Stormfront was injected with was different than the blue compound V that we are used to seeing. The formula has undoubtedly changed over the years and aging I'm guessing is something they engineered into it as a fail safe. I am also fairly certain that Homelander has a secret vulnerability and that's why Stan Edgar isn't afraid of him. Something like Black Noir's tree nut allergy but something that isn't totally obvious. I'mwilling to bet that Stan is the only one that knows his weakness.

In my opinion, SB and SF are very early test subjects for V which is why they got gifted with the slow aging. Vault changed this in the formula to create replaceable heroes.

They took the anti-aging out of the compound v that they gave homelander when he was a boy to use as a the security measure against him, because they knew what they were doing to him was wrong, and they were probably gonna create a monster.

I think it may be due to exposure to Solder Boys Uncontrolled Power. When he had 3-1 Fight against Butcher, Hughie and Soldier Boy, he was held down for a long time and exposed to some form of Radiation. As a result this may have a slow decline on a detrimental level on his Super Cells.

My take is that since the original formula for V was the purest it, somehow stopped the aging process. Since it was probably too expensive to make, the current iteration was developed and HL was genetically engineered to possess more than any known Supe. Because he was genetically modified from “conception”, he still needed to grow and develop so the ability to age was a must. He also stress tremendously which can exacerbate the effects of aging.
