This scene was not written with medical expertise. Superinflating an already properly inserted bladder catheter might rupture the balloon inside but would not cause pain since it would be inflating inside the bladder with plenty of room to spare until it popped. The balloon is small and is only meant to nest the catheter at the bladder neck.

An air embolism would require significantly more than a 10cc injection. An air embolism to cause a massive PE would theoretically require more like 50+ mLs of air or at least a possible minimum of like 30 mL.

The balloon of the catheter is in the bladder. No problem when it snaps. Reacher first had to pull the ballon in the urethra… He didn’t.
And 5 or 6 cc air will not give an embolism.

Having had a catheter post surgery and knowing the discomfort it causes…this made my skin crawl like never before.

Meanwhile, a perp who thinks Reacher is going to hurt him doesn't bother to raise hell and draw attention, how polite of him--but he's willing to withstand punishment to save his boss.
