Ultimate Form is definitely the top.
However, even though Final Form is pretty awesome, Double Form from KH3 is a completely broken beast as well. It definitely beats Final form for #2 ranking.

I'd give it to rage form. It stopped the complete X-Blade when Xehanort stripped Sora of his predominantly power "light".

Unity X form from when Sora fought the heartless storm in KH3 because, while temporary, killed hundreds of thousands of heartless in seconds and let him use pretty much millions of keyblades in the keyblade graveyard.
