Insane Painter can drop more than 5 on a coop level, I've seen people argue about whether or not he can drop up to 20 or not, and it's been far too long since I've done an insane 4 player Painter run, but I have seen him drop 15 in a 3p lobby. I'm pretty certain he can drop only potions and no food, as all food items are replaced by potions.

in insane mode the painter drops 5 potions PER PLAYER, so it's nice even if doing it with others
