I may be confused by what you are saying about plasma protein binding. Please clarify the following; Are you saying that medications need to be bound to a plasma protein to be distributed? That does not align with anything else that I have read. For instance, "Albumin is the most prevalent protein in plasma and the most important of the proteins to which drugs bind. ONLY UNBOUND (FREE) DRUG MOLECULES CAN LEAVE THE VASCULAR SYSTEM. BOUND MOLECULES ARE TOO LARGE TO FIT THROUGH THE PORES IN THE CAPILARRY WALL" Pharmacotherapeutics for Advance Practice Nurses and Physician Assistants edition 2. Or are you referring to a transport system called P-glycoprotein (PGP), a transmembrane protein that transports a large variety of drugs out of cells?

i still dont understand, if you ingest a pill and it is absorbed into hepatic portal vein and then carried to the liver (First pass), isn't that absorption then metabolism, then after it leaves the liver and goes throughout the body it is distributed? i am having a hard time understanding this.

Love your videos. You break it down into really clear facts and give a really clear concise explanation. Then the quiz at the end consolidates the learning. Thank you 😊
