I heard of a theory a while back that the "pestilence" that 049 speaks of is free will or individuality. After he was taken into foundation custody, 049 makes a comment about how there's "little trace of the pestilence here." The foundation is staffed with people with specific roles who are expected to follow strict rules and protocols and if you think of what constitutes as a "cured" being in 049's eyes (something with no individuality or free will) you can see where a small connection can be made.
It explains why he went willingly with the foundation and seemed to have no issues with Dr. Hamm initially. We don't know what exactly caused 049 to turn against Hamm since there's no transcript of their interaction prior to that but maybe Hamm inadvertently showed signs that he was more than just a foundation doctor following orders but a human being with free will and the ability to choose?
049 describes the pestilence as "an unspeakable horror, one that has shown its true face many times before and will again." With free will comes choice and with choice comes the ability to do harm or evil if desired. If there's no free will, there's no chance of evil being done and in turn, suffering ends (even at the cost of being an mindless, undead drone.) He himself said it wasn't a perfect cure but he still considers it a success regardless which I think is an important thing to note.
Anyway, I didn't mean to leave a fucking wall of text but I thought it was an interesting theory and wanted to share!
