For those that don't know, Kotaro kept going to the free balloons guy in different disguises (that he obviously saw right though) and saying things like "a red balloon for my younger sister", "a pink balloon for my mother". The guy assumed he was getting them to give to his family members but as he was walking past the park he found out he was getting them to MAKE those family members.

I think the saddest thing about this show (for me) is that, in some form or another, I've done the same things as this kid did.
The only difference is, in real life, people just laugh at you, mock you, and ignore you.
But because it's anime, and because he's drawn cutely, audiences are sympathetic towards him.

So while people hope that this boy finds happiness in his life...
There are thousands of kids just like him who just grow up bitter and alone.

I won the jackpot in life though. Am married, happy life now.
