For the final problem with the fundamental matrix, my professor taught that we take a definite integral from 0 to t with a different placeholder variable used within the integral during calculations. As it is a definite integral, that explains why the constant is left out, however I believe it yields a different answer than yours due the evaluation at 0 being subtracted.
