Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on the run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.

At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. Dr. Brewer provides services via telemedicine or in-person if you're in the Lexington, KY area. We find a lot of undiagnosed prediabetes or insulin resistance. Treating unrecognized risk factors like prediabetes allows reduction of risk and prevention of disease.

Your videos are definitely getting much more production/editing, but I think the editing is a little bit too aggressively done, sentence fragments a bit too jumpy, probably where pauses or uhms were cut, but I have to say I thoroughly enjoyed your older plain conversational videos too. Keep up the good work, your videos are literally life saving information to me, with familial hypercholesteremia, some pre-diabetic signs, and a personal weakness for sweets and deserts, but at 43 there is still time for my lifestyle interventions to make a big difference, including a switch to low-carb lifestyle for the past 2 years.

Even in the hospital "sudden" cardiac and other patient crisis events are usually precipitated by hours or even days of condition deterioration as indicated by vitals, labs, mentation, and/or complaints. It's often when the staff, both nursing and medical, fail to recognize and intervene when the patient is on the outer rim of _circling the drain_ that patients progress toward those "sudden" events and their condition spirals out of control.

The old fashioned treadmill stress test and the new chemical stress test didn't work for me. A simple questionnaire about what and when do you eat would have been far more beneficial.

The city just oiled my street again. They do it every two years. In the short run it is cheaper than concrete. It allows them to kick the can down the road. You know the oil causes all kinds of problems with your health, but I also worry if the chat is obtained from sources with radioactive dust. I told the Mayor you will pay a much bigger price by not doing it right the first time. She told me I didn't know all the facts and I was wrong. The Mayor has now been on sick leave for about a year.

I'm eating a large bowl of baby spring mix with lots of supposedly good stuff. The spring mix use to say triple washed, but now says thoroughly washed. I'm wondering if the company went to using "Victory Water", a chemical that companies add to water that washes and dries produce. You add one tablespoon per ten gallons of water. And of course the FDA says it's safe for Americans.

Thanks for the informative video Dr. Brewer. I fully agree that stress tests to evaluate heart disease risk are next to worthless. However, do you feel that they are a good tool when evaluating aortic stenosis? I'm 53 and was diagnosed with moderate aortic stenosis a year ago. Unusual at my age, except that I was also diagnosed with bicuspid aortic valve (BAV) and having aortic stenosis at 53 is very common for those of us in the 2% of the population born with BAV.
My father was given a stress test after his CAC score came back at 900 and his cardiologist used the stress test to clear him. I felt his stress test was meaningless, for the reasons you point out. However, I felt with my stress test to help evaluate my aortic valve function was different- not so much to "clear" me but used to see if symptoms could be elicited under stress and then used in conjunction with an echo to determine if it was time yet for my valve to be replace. Fortunately, the answer for me was no. Do you believe it a useful tool to help evaluate aortic stenosis?

6:46 This sounds very similar to chemotherapy; they keep doing chemotherapy and chemotherapy rarely cures cancer. Why do we do it? Because, it makes people a of money.
