Extensor Muscles Of The Thumb - Everything You Need To Know - Dr. Nabil Ebraheim

Dr. Ebraheim’s educational animated video describes the anatomy of the extensor muscles.

There are three extensors of the thumb: the extensor pollicis longus, extensor pollicis brevis, and abductor pollicis longus. The three tendons inserted into the thumb are abductor pollicis longus, extensor pollicis brevis and extensor pollicis longus.
The extensor pollicis brevis – shorter. It comes from the radius and inserts into the proximal phalanx of the thumb.
The extensor pollicis longus – longer. It comes from the ulna and inserts into the distal phalanx of the thumb.
The abductor pollicis comes from both the ulna and radius and inserts into the base of the 1st metacarpal bone.
The extensor indicis tendon may be transferred to replace a torn EPL tendon. The patient cannot extend the thumb due to a ruptured EPL tendon. The extensor indicis tendon may be used as a tendon transfer from its insertion into the index finger to restore the function of the thumb due to a ruptured extensor pollicis longus tendon.

when you explain things, it makes it SO CLEAR and the graphics make me understand it even better. I used to be at a loss on all the thumb extensors but your video makes SENSE and it's all crystal clear. Thank you so much for making these videos and posting them!!

Had EIP to EPL done... ( rt. wrist was chopped down on by glass)... couldnt move right thumb. After few years, started to experience pain in the thumb but more pain in the right forearm near elbow! Its now 5 years removed from surgery and pain is getting more intense in the forearm near elbow(outer)... The index tendon used to restore thumb movement, are you using the thumb's extensor longus muscle or the index finger muscle??? Im in construction and use my hands alot... also im right handed. Feels like something going to snap in forearm... It gets swollen after use in forearm... thumb is sore but forearm is much worse.... I need to be educated in what I can do to lesson the pain....

Thank you so much for this information. Cut my thumb yesterday and worried about EPL BEING SEVERED

I had a sprained thumb that never healed properly. It seems to relate to the extensor which becomes painful like a bruise that doesn't go away. I figured that the pain will heal on it's own. Unfortunately, the thumb is used often and wearing a thumb support glove does help but annoying for some chores in the house.

Is there a more favorable surgical outcome if the Pollicis longus tendon is partially cut vs completely severed?

Now I know which tendon got severed in a lacereation injury. I have use of my thumb, but it is at a severly wierd angle. My extensor pollicis brevis is gone. They never did surgery. I have no pain aside from the nerve damage thats also there.

Great👍
I have dislocation of extensor pollicis brevis tendon
And my doctor can't believe me
His diagnosis is simply:
De Quervain tenosynovitis
And he wants to do surgery to release it
I can see that the tendon is unstable and the inflammation is not an issue
What's your advice doc

Is it possible to purposefully stretch/ train my EPL to allow the distal phalanx to bend in a more 90° angle...at 60 years of age? Trying to play Jiminy Hendrix "thumb over the top" baseline chords...my old thumb just doesn't bend like that...yet? Any excessive suggestions would be appreciated.

Recently got a partial transection on this tendon near the phalanx. Still can’t extend it yet. So doctor put it in a splint & we’re gonna have to see how it’s healed about a month from now. If I remember this comment I’ll try and update.
