In-home health visits are available for those who qualify.

We understand that you may need extra care to support your medical needs. That’s why we offer a service called an in-home health visit to members with certain conditions or who are taking specific medications.

In-home health visits give you the opportunity to enjoy a focused, uninterrupted conversation with a nurse practitioner, in the comfort of your own home.

After the visit, we’ll share this information with your primary care provider and any specialist you’re seeing to make sure you’re getting the best care possible.

If you qualify for in-home health visits, you will receive a letter or phone call with information about the program and how schedule an in-home visit, if you like.
