X-ray Stages of Knee Arthritis

The video will go over x-ray stages for knee arthritis. Doctors often use x-rays to assess the severity of arthritis using the Kellgren and Lawrence system for classification of osteoarthritis. This is also called the KL grading system for osteoarthritis. It’s graded on a scale from 0 to 4 with 0 being no signs of osteoarthritis and 4 being bone on bone arthritis.

Kellgren and Lawrence classification.

0: A normal Knee
1: Small osteophytes
2: Osteophytes + first signs of narrow space
3: Deformation at the end of the bones, narrowing of the joint space and sclerosis
4: Severe steoarthritis

I have grade 3 on medial femoral condyle rather extensive area (arthroscopic imagines show even isolated bone exposed so grade 4!!!) could barely walk a year ago, having trouble sleeping because of pain at 57 years old. Followed Dr. Peng's videos and got hyaluronic acid injection also started oral hyaluronic acid (liposomal form!) at the same time, also take ASU (avocado soybean unsaponifiable) supplement, after a few weeks was able to return to tennis. Then a few months later after watching Dr Peng's PRP videos, got 3ml of PRP (with Xerthra kit, 15ml blood draw), takes time to fully take effect but was playing higher level SINGLES tennis again SIX days a week, 2 hours each session and pain free, some mild discomfort/stiffness after sitting long time or sleeping. ANYWAY yet another great video explaining reading the xrays....as a layperson/google search doctor I had no idea how to read my xrays....NOW I know what to look for!!!! SO SO SO GRATEFUL I didn't get sucked into the old school treatment of cortisone shots!!!! For anyone on a budget I would suggest trying oral hyaluronic acid (liposomal form!!!!), only costs about $20/month, I turned a couple of friends onto it and they say they notice a difference with that alone.

Hello Dr. Peng, does the patient feel pain or early morning stiffness in their knee in stage zero osteoarthritis? After X-ray of my right knee which shows normal spacing between joints, I was told I am having osteoarthritis. I am 35-years old and I teach at university.

I'm stage 4 and tried everything byt the tkr. I'm not thrilled about orthopedic recommending surgery.

It just happened to me last month when my mother-in-law was hospitalized and I had to take of her at the hospital for few days. I think during these days I strained my knee and then without knowing what happened, I started a very mild pain and some stiffness in the early morning. Now I am stressed and do not know what has exactly happened to my knee.

Ugh. I have bone on bone on both knees. But my right knee does not hurt very much, but the left one, boy howdy! Hurts.

I havs atage 4 knee osteoarthiritis medial compartment but he acid knee injections help. So theres no help for stage 4?

I work this is just a big inconvenient for me to have to be out for surgery I have goals I'm trying to meet life is all ready hard enough going out for knee replacenemt surgery is such a huge set back. Surgery doesn't always help either

Excellent video Doc & it's apparent that DJD is progressive and not good long term. Have you had any luck with PRP injections & at what KL grade would they not be effective? Or does it just depend on patient response? Thanks.

I got both knees now its causing me to not walk with pain all day and everyday how can i wake up with pain😢pain pills dont work they gave me my second injection and now i walk crucket not normal

I am now struggling with what to do about my knees. My xrays for 5+ years have noted the Osteophytes and mild to moderate narrowing of the space. What role might each of the two play in the pain? I want to use PRP, but if the spurs are causing 50% of the pain, the prp is not going to dissolve them (i assume). I have had kidney stones and the last one they had to use a laser with fiber optics to blow it away. Too bad you guys can't do that with a pulse laser to jack hammer the spur away, or can you?
