The Internal Revenue Service has an Office of Professional Responsibility, also called O.P.R..

This office is responsible for handling misconduct by tax professionals. O.P.R. reviews complaints, and takes actions when action is appropriate.

Use the I.R.S. Form 14157, with title “Complaint: Tax Return Preparer”. This form is used to report a tax return preparer for fraudulent tax return actions. You would complete this form, and attach all supporting documentation. The form and attachments should be sent directly to the I.R.S., either by mail or fax. The 14157 Form’s instructions provide the most current address and fax information.
