Easy Peasy Pharmacology brings a video on Anticoagulants, focusing more particularly warfarin and the direct acting oral anticoagulants - DOACs - such as dabigatran, apixaban, rivaroxaban and edoxaban.

In order to better understand their mechanisms of action it is vital to quickly visualize the clotting cascade where there are two possible pathways: intrinsic pathway and extrinsic pathway. Ultimately they converge into one common pathway which leads to formation of fibrin that provides stability to the clot.

Warfarin aspects such as regular monitoring of INR, narrow therapeutic window, drug interactions (drug-drug and drug-food interactions), as well as variability of response are covered on this video.
When it comes to the newer class of oral anticoagulants, DOACs, we explore some of their advantages when compared to warfarin and simultaneously give emphasis to certain aspects to bear in mind - they still present relevant drug interactions with other drugs and need alert monitoring of renal function as well as for signs of suspected bleeding (heavy menstrual bleeding in women, red or brown urine, bloody or dark stools, pains and swelling, blurred vision and other signs of anaemia).Dabigatran is a pro-drug - we will explain a bit about that too!
