- Improperly using beds to mine for ancient debris
- Entering a bastion
- Not carrying fire res when you go ancient debris mining (underground lava pockets burn the player)
- Getting knocked off a platform into a lava ocean by hoglins
- Not wearing gold around piglins
- Not wearing Soul Speed on boots (the souls trapped in soul sand drag you under)
