There was a head on collision during the filming of the smoke in the car scene. The stuntwoman, Heide Von Beltz was left a quadriplegic.

The smoke screen scene starting at 1:26 is more tragic than you know. Heidi Von Beltz, a stuntwoman, was in a DB5 for that stunt, when it went horrible wrong when a van turned into the path of the DB5 and crashed with it head on. The DB5 wasn't outfitted with seatbelts, and Von Beltz became a quadriplegic for the rest of her life as a result of the accident. As you can see from the footage, the crash occured right outside Las Vegas. I'm pretty sure the footage we see here was right before the crash. What I will never understand is why they needed a stunt double at all? Why didn't they just use a dummy in the passenger seat with a wig? They may as well have, because it's clear that the DB5 changes throughout the film. Note how the wheelbase changes, as well as the hood scoop. So, continuity errors weren't important, but they wanted a real person to be in the car without seatbelts for a stunt, which resulted in her being parayzed from the neck down for life, because a dummy in the passenger seat with a wig wouldve made no sense whatsoever. Tragic.

So sad that Heidi was left severely disabled due to the stunt at the end of this clip. Shows how basic health and safety was. Having a car which at the time of filming needed major repairs drive into oncoming traffic while full of smoke, a stupid thing to do.
