Hospital readmissions is a prime quality measure now shared across hospitals and home healthcare. Nationally, hospital readmission rates have been publically reported since 2007, with minimal improvement. Readmission rates are critical to home health agencies striving to excel at star ratings, value based purchasing, and contracting with referring hospitals, physicians, and payers. Why are readmissions so important? What do we know about who is readmitted, when, and why? What actually helps keep people at home? This session will review what is known about readmissions, the impact of readmissions as a quality measure, and highlight future opportunities for improvement.
