Chaos is the main reason. The Imperium created entirely new classes of battleship at one point that worked mainly as carriers and long range support firing vessels and they all fell to Chaos.

There's also the fact that, while they do create new weapon systems, they're generally inferior to their Dark Age counterparts, so yes, they can make plasma macro canons, but that communications tower that's able to send signals through even the Nid's interference is absolutely irreplaceable.

It really makes Belisarius Cawl a much more endearing character, that's not to mention that The AdMech codex specifies that not every Forgeworld considers innovation outright Heresy - Ryza being the example given. Every Forge has it's own version of the Omnissiahian Creed, with some hewing quite closely to Martian orthodoxy, and others straying a hair's breadth from being Hereteks

And quite often the designs are simply lost. Those maybe two dozen Emperor Class Titans? Yeah, that's all there is, no more can be made and every loss is permanent.
The Adeptus Mechanicus, then the Mechanicum, sees machines the way it does because during their fight for survival on Mars during the Age of Strife, keeping machines running was the key to survival. You didn't tinker with the water recycler or oxygenator to try and make it better. You kept it running and prayed that it kept working. Literally. Do that for hundreds, if not thousands of years and you've got a cult on your hand that worships machines and abhors innovation.

Desperation breeds innovation and progress. It seems the Imperium has not been desperately pushed to the point that they **HAVE** to change their ways in order to survive. They **WILL** or **DIE**. It's that simple.

Another reason is that while within the setting Imperium's technology development had stagnated...said tech is still absurdly advanced and powerful.

They still possess a stockpile of weapons that can scour life away from a planet. They still possess the means to make augmented super-humans that are a cross between mobile artillery and elite infantry.

Also needs to point out that the ritualization and seemingly irrational reveration of old technologies also further reflect the dependency on, and the loss of knowledge.

The Mechanicus, with all their ingenuity, is only a mechanic trying to do the engineer's job.
