I'm getting a 12v rail failure after doing this. What does that mean. Also, with a surge tube in place, what could have caused the power supply to fail?

I keep getting this message: "Override not toggled. The operator override was not opened and closed." This is for a machine I got today. Opened and closed that thing so many time!
