Serum Sodium - chloride . Normal is 36 +/- 6. So if > 42 it’s an additional metabolic alkalosis is present and if it’s less than 30 an additional metabolic acidosis is present. The way I interpret in the ICU is : Primary disorder- look for compensation, followed by Anion gap and then the above equation. Works all the time.
