I used to work at Fantasmic this is true and can happen for multiple reasons. The cauldron is brought up to the stage via an elevator lift. The hag performer has another role and has to run from that, down to the basement under the stage, get changed into the hag costume and get into the lift before it rises. If they don't make it in time they have to send the lift when it's time whether the hag actor is inside or not. There is also a backup for if the lift isn't working. You'll see the Queen simply walk in from offstage and perform without the cauldron.

The queen does not suddenly put a wig on in these situations. She wears a wig under her cowl and crown every performance in case she has to do one of these backup situations. She simply removes the crown and cowl which exposes the white wig underneath.

There are multiple contingencies for every possible situation and technical problem to be sure the show can continue without stopping. It's very rare the show will be halted even under the very worst of technical malfunctions. Actors have been injured without the audience being too much wiser to the situation because the show just continues and you may only notice that a small part has been changed.
