Practice Nurse Led Innovations to Improve Integration for Patients with Complex, Chronic Conditions.

This is particularly relevant for individuals with complex and long-term conditions, as they and their families regularly interact with multiple healthcare providers. Improvements in integration of care between providers requires support and active participation from all components of the system; executive level as well as those working on the ground. This project demonstrated the benefits that can be achieved by training and empowering practice nurses to test innovative ideas to enhance communication across the system. By focusing on a cohort of patients with chronic and complex needs, systems level changes to improve the quality and timeliness of information could be tested before wider implementation.
