What if Dooku was overthrown by someone in the Separatist leadership who finds out about Palpatine's plan and Dooku is working for him but this leader still wants the CIS to remain independent from the Republic, what would happen?

I think you are overlooking the fact that a few of them could have just been too well hidden and sid and vade just couldn't find them. I mean the Galaxy is a big please and lots of places to his and you can't wipe out what you can't find.

One thing you neglected to mention was that the CIS was started and promulgated by business tycoons who were intent on maximizing their profits. If Outer Rim holdouts were allowed to survive they would quickly become redoubts for these same tycoons who wanted to save their businesses from Palpatine's nationalization of all industries. The businessmen would pour their fortunes into making the holdouts into zones of free trade. Any surviving CIS warships and naval personnel would quickly become the new Trade Federation. They would promote the holdouts as competition to the Corporate Sector. Smugglers like Solo or Calrissian would find homes there, and they would tell their friends. Yes, the Imperial Navy might try to starve them, but their own merchant fleet to supply them, this would be a costly and ultimately useless gesture. Simply put given enough time and resources, Separatist Holdout could quickly become sizeable star empires of their own.

I was under the impression that a few CIS holdouts where still holding out even up to the destruction of the second death star

I wonder if that's what the "corporate sector" might look like in the new canon? Seperatists that are nominally under the empire, but practically function under corporate oligarchs.. since they publicly bend the knee it wouldn't greatly change public perception of the empire, but internally it would basically be CIS.

What if grievous fled utapau, and went to mustafar a few days after Vader, reactivating the droid army and becoming the new leader of the CIS. With Dooku and the council dead, grievous would be the sole commander of the army. He could use it to his advantage to launch attacks from behind republic lines, since many offline droids would be ignored by cleanup crews and scrappers for at least a few days

Now that there's a new canon for SW, I think that'd be interesting to see a remnant of the CIS or a new group of CIS to challenge/work with the Empire or maybe try to seize power after Sidious' death.

What if the Ruusan Reformation never happened? No Limits to navy or army size. Would be cool to see huge Dreadnoughts during the Clone Wars or maybe a side faction like The Hutt Emprie having a huge Military. And #CIS

Well the Galactic Empire didn't really control the whole galaxy. Let's not forget they let the Hutt's rule a large portion of the galaxy as well. Granted they made deals with, because taking on the Hutt Cartels could be a losing cause.

I actually came up with an idea for a fan film about a separatist hold out planet. It stayed free from the Empire and Republic when the 1st Order came around.

If any holdout still existed they would have to hide at abandon factory worlds(hypori) futher into outer rim or the unknown region . Example would be when poggo the lesser Lt. Seized Mustafar in the EU canon. Only way to create or continue the fight would be to buy it off the black market, favours with the corporate sector, or establish new factories on other worlds.

Lingering holdouts as controlled opposition would be very useful in the early years of the Empire. They would serve as further justification for military expenditures.
