The girl had died with his vampireblood in her system so she had to become a vampire and when she did she became sired to him and she would do whatever he wanted to make him happy but the sir bond doesn't affect her feeling for him so the other girl realized what was happening she knew they were sired everything added up after all Elena was sired to Damon and it started getting more and more obvious.

A sire bond is making someone loyal to you, but only if they had feelings for you as a human. Elena had feelings for Damon when she was a human, so, she she got turned into a vampire from Damon’s blood, she became sired (loyal) to him. For hybrids, they are automatically sired to Klaus even if they did not love him before they were turned.

Klaus was the first to figure it out. But obviously he had a lot of experience being 1000 years old and smart but Caroline was the smartest in their group for sure. She was always the first to figure things out and put things together. Its sad how some people under-estimated her like her mom did in the beginning. Me and my friends have talked about this and the fact that Damon and Stefan were involved in the supernatural world WAY before Caroline and how we expected them to be more quick in figuring things out (like care), they weren't bad they just thought A LOT of themselves. Caroline is honestly one of the best characters if not the best character (+ Katherine and bex + Klaus ) on Tvd.
