As a crisis counselor attached to an ED I can confirm this is exactly what it is like.

Obviously (for those who don't work in the field) some of the verbiage here we literally will quote as something the patient has said to make sure that's documented, but other stuff as it relates to our work we have to be careful about since patients can request to see their notes if they want.

It's actually pretty frustrating because all of the medical jargon we have to stick to (and each hospital is a bit different) doesn't always paint a detailed picture. Saying "The patient was pissed no one called their aunt despite requesting to do so 5 times in 14 hours" paints a useful picture of the situation where as "Patient was emotionally dysregulated and expressed concerns about contacting their aunt" doesn't capture it. Which doesn't set up the next RN/Dr/CW to understand what the hell is going on.

I get why we have to do it, but it also sucks. Which is why having decent working relationships with others in the ED is soimportant, because what we get to say to each other is way fucking different.

Also, this video had me busting out laughing. The only thing that's missing is the rummaging back and forth through paperwork because you're so fucking tired you can't remember something basic - like the patient's damn age.
