How do doctors, who have previously been good doctors, find themselves down a path like this?

Specifically on the issue of deaths attributable to medical errors, there are a number of articles out there that are of interest when it comes to analyzing the problems with determining how many deaths are actually attributable to medical error.

One is an August 2019 article by Dorion, Hinds and Thompson, appearing in Healthy Debate and available online.

One issue is that in order to classify adverse outcomes as the result of medical error, one must have an idea of what ought to have been done in these cases, as opposed to what was actually done, and to also be able to determine that that error caused the negative outcome.

While I support the recommendation that the BMJ article that famously estimated 251 000 deaths by medical error made, namely that reporting criteria be adjusted so that we can have a more accurate picture of the toll of medical errors, it seems there are good reasons to think that their estimate is not accurate.
