Adam Smasher is 96% machine, but that's not really abnormal for Cyberpunk. At least 2020.

I think it has something to do with him canonically using the Dragoon, but customized and with none of the requirements like a secondary pilot or being dosed, so he can take the raw input that normally requires two people to handle and can do it without the drugs that keep the person in it from snapping.

my interpretation is in 2077 being a full borg simply isn't fashionable and probably considered generally impractical, these examples are from 2020 (in universe). the difference between being a full borg and just having a shit ton of cyberware is very minimal after all.
I suspect becoming a full borg is a lot less mentally stressful then some specific cyberware (e.g the sandev or anything in the game with a high cyberware capacity), stuff like the gorilla arms which are fundamentally just stronger replacement arms don't have a high cyberware capacity after all, scaling that up to a full body likely isn't that mentally stressful. quick reminder Lizzy Wizzy is also a full-borg (though she is implied to be a high functioning cyberpsycho).
Smasher isn't special because he's a full borg (other than it being rarer in 2077), he's just able to handle more extreme cyberware much better than the average person.

I wonder if lizzy wizzy qualifies as close to a full borg? Shes got the fully metal body to the point of kinda slowly slipping into cyberpsychosis, and there was a shord somewhere describing a performance she put on where she drank some kind of poison and then had a bunch of ripperdocs come on to replace all of her damaged organs and save her life before she gets back up and keeps performing. Strikes me as at least 90% borg at that point, no human guts, metal bosy, but definitely still a human brain, hence the in game discussion of her manager wanting to use soulkiller on her to get her engram. I like the concept of someone fully borg but not in the mercenary line of work, conceptually intersting
