Ellen was hurt but did not have a permanent spinal injury. That was Linda Blair for being thrashed around on the bed in a back brace that came loose and the guy that was operating it didnt know, so he continued on with the scene.

In her book, Ellen Burstyn shared a funny little anecdote from the filming of this scene, unrelated to the injury. Linda “striking” Ellen was acting, but in one particular take Linda did accidentally hit her, and she panicked and began apologizing profusely like any polite and kind young girl would, which was funny to Ellen coming from behind all of Linda’s horrible demon makeup.

As for Ellen’s injury, it had nothing to do with Linda pretending to hit her. Ellen had a wire attached to her which the stunt technician pulled back too aggressively, on William Friedkin’s instructions.
